عنوان فارسی بررسی تاثیر برنامه تنظیم ساعتی مصرف مسکن بر کیفیت خواب و تغییرات رفتاری کودکان پس از جراحی: یک مطالعه کارآزمایی بالینی.
چکیده فارسی مقاله مقدمه: عدم کنترل مؤثر درد پس از جراحی می تواند باعث اختلال خواب، کاهش دریافت مایعات و بروز تغییرات رفتاری مثل بی‏قراری، کج‏خلقی و کاهش فعالیت و بازی شود. این مطالعه با هدف تاثیر برنامه تنظیم ساعتی مصرف مسکن بر کیفیت خواب و تغییرات رفتاری کودکان پس از جراحی انجام شد. روش کار: در این کارآزمایی بالینی 68 کودک 6 تا 12 سال بستری جهت عمل جراحی تانسیلکتومی در بیمارستان بعثت و شهید مطهری همدان سال 1392، به همراه والد مراقب انتخاب و به ‏صورت تخصیص تصادفی بلوکی به دو گروه کنترل و آزمون تقسیم شدند. برای گروه آزمون مداخله شامل آموزش برنامه تنظیم ساعتی مصرف مسکن پس از ترخیص انجام شد. ابزار گردآوری داده هاچک‏لیست ثبت وقایع در منزل بود که توسط والدین تکمیل شد. داده‏ها توسط SPSS نسخه 16، با آزمون­های تی مستقل، کای دو و آنالیز واریانس با اندازه­های تکراری تحلیل شدند. یافته‌ها: بین تفاوت میانگین نمرات کیفیت خواب شبانه کودکان گروه آزمون و کنترل در روز اول پس از ترخیص تفاوت معنی‏ دار آماری مشاهده شد (008/0p=). بین تغییرات رفتاری فقط در متغیر کاهش اشتها در روز اول تا سوم پس از ترخیص تفاوت معنی دار آماری مشاهده شد (00/0p=). نتیجه‌ گیری: آموزش پیروی از برنامه تنظیم ساعتی مصرف مسکن در سه روز اول پس از جراحی های سرپایی مانند تانسیلکتومی می تواند از بروز تغییرات رفتاری نامناسب در کودکان جلوگیری و نیز باعث افزایش کیفیت خواب و اشتهای آنها شود.

عنوان انگلیسی The Effect of Around-The-Clock (ATC) Analgesic Administration on the Quality of Sleep and Behavioral Changes in Children after Surgery: A Randomized Clinical Trial
چکیده انگلیسی مقاله Introduction: Lack of effective control in postoperative pain can cause sleep disturbance, decreased fluid intake and incidence of behavioral changes, such as restlessness, irritability and reduced activity and play. Therefore, this study was performed with the aim of determining the effectiveness of Around-The-Clock (ATC) analgesic administration on the quality of sleep and behavioral changes in children after surgery. Methods: In this clinical trial, 68 children, 6 to 12 years old, admitted for tonsillectomy with one parent, were selected and randomly divided to control and case groups. For the case group, an intervention was performed by the Around-The-Clock (ATC) analgesic administration training after discharge. The data collection tool was a home dairy that was completed by parents. Data were analyzed with SPSS 16 by repeated measures, post-hoc, independent t and chi-square tests. Results: According to the independent t test, there were significant differences between children of case and control groups in average sleep quality scores on the first day after discharge (P = 0.008). According to chi-square test, except for appetite (P = 0.00), no significant differences were shown between children of the two groups in the other behavioral changes. Conclusions: Training of Around-The-Clock (ATC) analgesic administration in the first three days after ambulatory surgery, such as tonsillectomy, could prevent inappropriate behavioral changes in children and increase the quality of their sleep and appetite.
