بررسی مقایسه ای اثر نسبت تاج به ایمپلنت بر روی میزان تحلیل استخوان مارجینال اطراف ایمپلنت در نواحی خلفی ماگزیلا و مندیبل |

مقدمه و هدف: با توجه به رشد روز افزون استقبال بیماران برای استفاده از درمان ایمپلنت لازم است که کادر درمانی با اشراف کامل به فاکتور های موثر در پیش اگهی و موفقیت درمان از شکست های قابل پیشگیری جلوگیری نمایند. هدف ازاین مطالعه تعیین تاثیر نسبت های مختلف طول تاج به طول ایمپلنت (C/I Ratio ) در نواحی خلفی ماگزیلا و مندیبل به عنوان یکی از اصول مهم بیومکانیک بر روی تحلیل استخوان مارجینال اطراف گردن ایمپلنت (MBL) است. روش کار : این مطالعه بصورت یک مطالعه همگروهی آینده نگر اجرا گردید. در طی این مطالعه بیمارانی که در طول سال (93- 92) در یک مطب خصوصی تحت درمان ایمپلنت قرار گرفتند انتخاب و رادیوگرافی های 6 ماهه و 12 ماهه دیجیتال موازی ایشان جهت بررسی تاثیر نسبت تاج- ایمپلنت در نواحی خلفی ماگزیلا و مندیبل توسط کولیس دیجیتال بررسی گردید. همه بیماران مورد ارزیابی کلینیکی و رادیوگرافیک قرار گرفتند و نتایج حاصله توسط آزمون های آماری t-test و رگرسیون خطی چندگانه مورد بررسی و تحلیل قرار گرفت. نتایج: در مجموع 48 فیکسچر از سیستم SIC در 18 بیمار شامل 28 عدد در ماگزیلا و 20 عدد در مندیبل مورد ارزیابی قرار گرفت. آنالیز آماری سطوح استخوان مارجینال اطراف گردن ایمپلنت ها در رایوگرافی های تهیه شده، تفاوت معنی داری را در رابطه با تاثیر نسبت های متفاوت C/I Ratio بر روی تحلیل استخوان مارجینال اطراف ایمپلنت نشان نداد (094/0 = P) ، همینطور بررسی مقایسه ای نواحی خلفی ماگزیلا و مندیبل حاکی از عدم تفاوت تاثیر مقادیر متفاوت C/I Ratio بر روی تحلیل استخوان مارجینال بود. نتیجه نهایی: مطالعه حاضر نشان داد که در بررسی ایمپلنتهایکارگذاری شده در نواحی خلفی دو فک با دارا بودن نسبتهای تاج به ایمپلنت از87/0 تا 55/2، میزان تحلیل استخوان مارجینال بررسی شده از نظر آماری تفاوت معناداری دیده نمی شود که حاکی از عدم ارتباط تحلیل استخوان مارجینال اطراف گردن ایمپلنت با نسبتهای متفاوت تاج به ایمپلنت و ناحیه درمان است(05/0P>). |

Comparison of the Influence of Crown to Implant Ratio on Marginal Bone Loss around Implants in Posterior Areas of the Maxilla and Mandible |

Introduction & Objective: Due to the patients’ growing interest in the use of dental implants, medical staff should be completely aware of treatment success and prognostic factors to prevent failures. The purpose of this study was to evaluate the effect of different crown to implant ratio (C / I Ratio) in the posterior areas of the maxilla and mandible as one of the most important principles of biomechanics on marginal bone loss around the implant neck (MBL). Material & Methods: This study was conducted as a prospective cohort study. In this study, patients treated at a private dental implants office during the years 2013-2014 were selected. Parallel digital radiographs 6 months and 12 months after loading the implants were measured to evaluate the effects of prosthetic implants on posterior areas of the maxillary and mandibular crowns by a digital caliper. All patients were evaluated radiographically and data were collected and analyzed by SPSS 16 with non parametrical tests. Results: A total of 48 fixtures of SIC Implant System in 18 patients, including 28 pcs in maxilla and 20 pcs in mandible were evaluated. Statistical analysis of marginal bone level around the implant neck in radiograpy taken showed no significant difference in the effect of different C/I Ratio of marginal bone loss around implants (P=0.094). Comparison of posterior areas of the maxilla and mandible also showed no difference between the effects of different amounts of C/I Ratio of the marginal bone. Conclusion: The present study showed that the marginal bone loss was not statistically significant after the placement of implants in the posterior region of the jaw with a different range of crown – to- implant ratio from 0.87 to 2.55. Thus, it seems that there is no relationship between marginal bone loss around the implant neck with different proportions of crown - implant and with the location of the treatment (P>0.05).
