مقدمه: حوادث یکی از مهمترین علل بروز خسارات در یک سازمان میباشد که به واسطه سلسلهای از خطاهای اغلب جزیی و کم اهمیت یک فرد و یا یک مجموعه رخ میدهد. ممکن است هرکدام از خطاها، پیامدهای جزیی داشته باشند اما با ترکیب اثرات آن ها، مجموعه یا سیستم به طور تغییر ناپذیری به سوی پیامدی خطیر و فاجعه آمیز حرکت میکند. به همین دلیل این پژوهش با هدف شناسایی خطاهای انسانی منجر به حوادث با استفاده از مدل تجزیه و تحلیل فاکتورهای انسانی و طبقه بندی سیستم در یکی از پالایشگاههای گاز انجام گرفت. روش کار: مطالعه حاضر یک مطالعه گذشته نگر میباشد که در یک پالایشگاه گاز انجام شد.آمار حوادث از پالایشگاه گاز تهیه شد، ابتدا گزارش تحلیلی علل ریشهای (RCA) حوادث که از سال 1388 تا 1396 اتفاق افتاده بود تهیه شد و سپس با استفاده از چارچوب HFACS تجزیه و تحلیل گردید. یافته ها: بیشترین خطاها مربوط به سطح اول است یعنی خطاهای ناشی از اعمال ناایمن که در این سطح زیرگروه "تخطی" ، سطح 2 زیرگروه "محیط فیزیکی" ، سطح 3 زیرگروه "نظارت ناکافی" و سطح 4 زیرگروه "مدیریت منابع" بیشترین تعداد تکرار را در سطوح مذکور دارند. نتیجه گیری: نتایج مطالعه حاضر علتهای بروز حادثه و نقاط ضعف متعدد در پالایشگاه را آشکار ساخت. با بکار گیری همزمان کنترلهای مدیریتی و با ایجاد فضای آموزشی مناسب و بالا بردن سطح آگاهی افراد می توان به کاهش خطای انسانی در سازمان کمک کرد. |

May each error, Have minor consequences But with combination their effects, sets or systems are moving invariably to the serious consequences and disastrous. Therefore, this study aimed to identify human errors leading to incidents in a gas refineries using Human Factors Analysis and Classification System. Methods: This study is a retrospective study that was performed in a gas refinery. incidents Data of this study be provided from gas refinery, first root causes analysis reports (RCA) of incidents that occurred during the last 8 years was prepared And then were analyzed by using the HFACS model. Results: the maximum error of HFACS viewpoint Related to the first level i.e errors caused by unsafe acts that in the first level was related to the "violations", in the second to the physical environment, in the third level to the inadequate supervision and in the fourth level to the management of resources have a Most repeated in mentioned levels Conclusion: The results of this study showed the causes of accidents and several weaknesses In refineries. It can be helped to reduce human error in the organization by utilization at the same time administrative controls and by creating an appropriate learning environment and raise employee awareness. |
